Learning To Use The System

Get Started • Life Skills / Indicators • Consent • Form • Enter Data • Reports • Write Results • Plan Programs

The Life Skills Evaluation System has been created to assist you in evaluation.

  • Follow the step-by-step instructions on each page to create an evaluation instrument to use directly with program participants.
  • One goal of the Life Skills Evaluation System is to gather county and statewide data focused on life skill outcomes.  Use the statements listed in View Life Skills.
  • The Life Skills Evaluation System is linked closely to the Logic Model at the University of Wisconsin-Extension (UWEX), a program planning process designed to assist you in evaluating outcomes of your programs.

General Principles:

The Life Skills Evaluation System is designed to:

  • Assess only the selected nine life skills.
  • Measure outcomes of programs with six or more contact hours with the same audience.
  • Assess participants who are 6th grade and above.
  • Accept data from the life skills outcome indicators listed on this Web site. [Use outcome indicators as written.]

For a successful evaluation, make sure that you:

  • Use the permission statement at the top of the evaluation form as it's written.
  • Administer the evaluation face-to-face at the end of your program.
  • Assess program participants only, not observers of participants, such as parents of youth who participated.
